Hello and again welcome to Quilts Of Love. We are so very happy that you have decided to have a graphic quilt made for your child. The requirements to have a quilt "stitched" by one of our Quilting Angels is that the child has an illness or condition that is life long, life threatening, or a child is terminally ill.  Child should be 16 or younger when Quilt is requested or with a mental condition that makes them childlike (i.e. Down Syndrome or severe brain injury). We reserve the right to decline to make a Quilt of Love.

Please read the following carefully BEFORE filling out the request form. It will make the process go much quicker for you and for us.

1) If you know of a child who may benefit from a love quilt but is not a family member you may fill out the request form but must give the e-mail address of a parent or immediate family member. We must have permission from a parent or grandparent to proceed with making the quilt. And please let them know you have done so. We would hate to mail a parent to confirm their child's quilt to be made and they not know what we are talking about. So please discuss this first with the child's parent in advance.

2) We will always need a current and working e-mail address for your child. This is the only way we can keep in touch with you and also will be the way we will let you know of special pages such as holiday pages that are celebrated here at Quilts Of Love. Please make sure that you whitelist (safe sender) our email address of quiltsoflove@quiltsoflove.com so that our email can reach you and not be lost in your spam folder.

3) Also please include web page address if your child has one. We will add the link to your child's quilt page so visitors to your child's quilt will also be able to visit his or hers home on the web and will be able to keep up with your child's progress. When adding your child's website our form please make sure to start with www. rather than http://www - this is because of the way our form is designed to reach us and the link be 'clickable'.

4) We require at least one photo of your child in order to make their Quilt of Love. If you would like your child's friends and family member's photos included in the quilt, please send photos by email to Quilts Of Love at the e-mail address listed below, or, if you or your child has a web page and photos are there that we may use in the making of their quilt, please let us know and we will need your permission to use these photos. We will need at least one photo of your children alone as their main photo on the quilt. Other photos will be added as space and design allow.

5) We are a Christian based group; however, we make Quilts of Love for families of ANY faith and no faith.
Holiday pages are mailed out at the appropriate time. If you do not celebrate traditional USA holidays please notify us of that and include in the message of the form. This is why we request to know your religious affliation if any on the request form. We do make Halloween pages ~ fun type - for the kids so if you would prefer to NOT receive the Halloween page, please just let us know when you send in your form. Other holidays we celebrate and make pages for are Valentines Day, Easter, Mothers Day, Fathers Day, 4th of July, Thanksgiving and Christmas. You can opt out of our mailings at any time.

6) All information is confidential and used only in the process of making a quilt to fit the specific needs of each child. We will never give out you or your child's e-mail address to anyone other then the Angels who will be creating your child's personal Quilt Of Love or sending them birthday wishes. Your child's last name will NOT be used on our site - only first name and initials.

7) Music is placed on each child's page. Please understand that with web pages, we must use a special format of 'music' called midi files due to the size of song files. We are normally unable to place specific songs from newer artists or groups because they are not available in midi format. However, we will attempt our best to find music that your child will enjoy. We do ask for their favorites on the form; just keep in mind that we can't put music from artists such as Hannah Montana or High School Musical as that music is not available in midi form. Most small children's musical likes are available such as Twinkle, Twinkle Little Star and other classic kids songs. For an older child, if you tell us their favorite artist, we can sometimes find a midi file based on one of the artists songs (but not all).

8) Please, keep your Email address updated with us if any changes are made. This is the only way we have to contact you with updates to quilts and holiday pages. We do know that from time to time you may have to change your address and sometimes it is hard to remember all who need to be informed. Just think of us as family and we are sure you will remember to let us know. All you need to do is use the e-mail address that is on each and every page here at Quilts Of Love.

9) A personalized poem just about your child will be included on the quilt page; these are written by our wonderful volunteer poetry writer who attempts to capture your child from the information on the form that you send in.

We welcome you to fill out the request form below. You will be contacted within two days by one of our Angels to confirm that you still want your child's Quilt Of Love "stitched" by our staff of Angel's who donate their services out of love and from the heart to bring love, happiness, and smiles to children's faces all over the world and especially to your child. Please note these are GRAPHIC only Quilts - not 'sewn' quilts. **** Once we email you, you will need to reply to that email in order for us to begin the Quilt. This is our 'confirmation' process that assures us we have the right email address to notify you when the Quilt of Love is finished and send you the link to it. We only hold requests for 2 weeks - if we don't hear back from you after our email to you, your request will be deleted****** If you have not received an email from our group within 48 hours of submitting a request, please email us at our main email of quiltsoflove@quiltsoflove.com

**We are unable to make Memorial Quilts for the general public due to our small staff of volunteers and limited server space. Thank you for your understanding.**
